Job summary

Infinite Home Care & Disability Services Pty Ltd in Sydney is seeking an experienced Community Worker to join our team on a full-time basis. You will support NDIS participants to achieve goals, build independence and participate in local community activities.

You will collaborate with families, support coordinators and health professionals, maintain accurate records, and use person-centred practices to promote choice, dignity and inclusion. NSW-based role with opportunity for development.

Qualifications

• Qualification in Community Services or related field.
• Experience with people with disability or community services.
• Understanding of person-centred practice and NDIS.
• Strong communication and interpersonal skills.
• Good written documentation and record-keeping.
• Current NDIS Worker Screening Check.
• Current First Aid and CPR.
• Working with Children Check, where applicable.
• Australian driver's licence and reliable vehicle.
• Ability to work across Western Sydney.

Responsibilities

• Provide person-centred support to NDIS participants based on their individual needs and goals.
• Assist participants with community participation, social and recreational activities.
• Support participants to develop independence and daily living skills.
• Assist participants to access community services, appointments and activities.
• Develop and implement appropriate support strategies in consultation with participants and relevant stakeholders.
• Monitor participant progress and identify changes in support needs.
• Maintain accurate progress notes, case notes and participant records.
• Liaise with participants, families, support coordinators, health professionals and other service providers.
• Identify and report incidents, risks and changes in participant circumstances.
• Promote participant choice, dignity, privacy, independence and inclusion.

Infinite Home Care & Disability Services Pty Ltd is a registered NDIS provider delivering person-centred disability support and community-based services across Western Sydney, NSW. We are seeking an experienced and motivated Community Worker to join our team on a full-time basis. The successful candidate will support people with disability to achieve their individual goals, develop independence and participate in their local community.

Key responsibilities

Provide person-centred support to NDIS participants based on their individual needs and goals.

Assist participants with community participation, social and recreational activities.

Support participants to develop independence and daily living skills.

Assist participants to access community services, appointments and activities.
