Job summary

Healthcarelink Support in Bunbury WA is seeking a Speech Pathologist focused on Community NDIS to deliver assessments, interventions, and planning in participants’ homes and community settings.

You will work autonomously within an Allied Health team, providing person-centred, holistic care and capacity building to help participants achieve their NDIs goals. A strong emphasis on collaboration and timely reporting is required.

Qualifications

• Tertiary qualification in Speech Pathology.
• Membership with Speech Pathology Australia.
• Experience in the relevant industry to undertake the activities listed.
• On-the-job training and relevant experience.

Responsibilities

• Manage a caseload of participants in home/community settings.
• Conduct assessments and recommend functional impact on eating and swallowing.
• Develop mealtime management plans with other health professionals.
• Assess and recommend AAC systems under NDIS and funding bodies.
• Provide capacity-building, client-centred care to meet NDIS goals.
• Report outcomes and recommendations in a timely manner.
• Provide training on AAC, mealtime management and communication.
• Promote health and wellness to improve outcomes for participants.
• Coordinate with families, referrers and other health professionals.
