Job summary

Chosen Family is seeking a Female Disability Support Worker to provide compassionate, consistent in-home and community support for a 61-year-old participant in North Sydney. The role focuses on promoting independence, meaningful activities, and social participation, with an emphasis on OCD and ABI-informed care.

A vehicle and driver’s licence are required. The position is casual and entry-level, offering flexible shifts on Monday through Sunday.

Qualifications

• Experience with Acquired Brain Injury (ABI), OCD or similar support needs is highly regarded.
• NDIS Worker Screening Check, First Aid & CPR Certificate are required.
• Valid Australian Driver's Licence and reliable vehicle for community access are essential.
• Strong communication and interpersonal skills with a calm, patient approach.

Responsibilities

• Provide companionship and meaningful conversation.
• Support a structured and predictable daily routine.
• Encourage participation in meaningful activities and community access.
• Drive the participant to appointments, activities and community destinations.
• Support independence at home and in the community.
• Provide calm reassurance during anxiety or distress.
• Complete accurate shift notes and documentation.
• Communicate professionally with the participant and support team.
