Job summary

Uniting AgeWell Echuca Home Care is seeking a dedicated Community Registered Nurse to deliver in-home clinical assessments, care reviews and plans. You will provide clinical guidance, support colleagues, and coordinate with allied health professionals to ensure responsive care in clients’ homes.

Applicants should have a Bachelor of Nursing with current AHPRA Div 1 registration and 3–5 years’ RN experience in community settings. Driver’s license and vehicle ownership are essential.

Qualifications

• Bachelor of Nursing with current AHPRA Div 1 registration.
• Minimum 3–5 years’ experience as a Registered Nurse in Community, CHSP or District Nursing.
• Endorsed for Continence Assessments and Prescription (highly desirable).
• Experience visiting clients in their homes and developing care plans.
• Familiar with My Aged Care portal and person-centered care.

Responsibilities

• Conduct in-home clinical assessments, reviews and care planning for clients in the East and Outer Eastern suburbs.
• Provide clinical advice and referrals to more specialized services as needed.
• Lead and supervise Care Advisors and Home Care staff in care delivery.
• Collaborate with local allied health professionals to coordinate home-based care.
• Deliver health education, social support groups and carer education.
• Support safe medication management and clinical goals through team coordination.

About the role:

This opportunity isPermanent Full time, Monday to Friday during business hours (0830 - 1635 - flexible)

Supporting our Home Care program based out of our Echuca Home Care Office, the Community Registered Nurse services Uniting AgeWell clients in Mildura and the surrounding region.

• In-home clinical assessments, reviews and care needs for clients in the East and Outer Eastern suburbs
• Provide clinical advice and support to clients and provide referrals to services where more specialized support/advice may be required
• Provide collaborative leadership, support and supervision to Care Advisors and Home Care staff
• Work closely with local allied health professionals to facilitate responsive care in our clients homes
• Provide health education and promotion, including social support groups and carer education
• Promote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management and achieving clinical support goals
• Office based position however flexibility is negotiable with organisation and client needs

About you:

This is the place for you if you have:

• Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration (required)
• Minimum 3-5 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing (required)
• Team player, flexible in your approach to work with a positive outlook
• Endorsed for Continence Assessments and Prescription (highly desirable)
• Experienced in actively visiting clients in their homes
• Experienced with evaluating and implementing care support plans
• Familiar with the My Aged Care portal
• A kind and caring heart, with a passion for person-centered care and advocating for clients
• Proficiency in decision making, critical analysis and on the spot problem solving
• Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions

• A current Working With Children Check employee card
• Current and ongoing flu vaccinations
