Job summary

Uniting AgeWell is seeking an experienced Registered Nurse to support our Echuca Home Care program. This permanent full-time role covers in-home assessments and client care across Mildura and surrounding areas, with office time and flexible scheduling to meet client needs.

You will lead care planning, collaborate with multidisciplinary teams, and promote person-centered age care in the community. AHPRA Div 1 registration and a vehicle are required.

Qualifications

  • Registered Nurse with current AHPRA Div 1 registration.
  • 3–5 years experience in Community, CHSP or District Nursing.
  • Ability to visit clients at home and coordinate care plans.
  • Endorsed for Continence Assessments and Prescription (highly desirable).
  • Own a registered and insured vehicle.
  • Strong communication and teamwork skills.

Responsibilities

  • Perform in-home clinical assessments and care reviews for clients.
  • Provide clinical advice and referrals for more specialized services.
  • Lead and supervise Care Advisors and Home Care staff.
  • Collaborate with local allied health professionals to coordinate care.
  • Deliver health education and promote social support programs.
  • Promote teamwork and model safe medication practices.
  • Office-based role with flexible arrangements depending on client needs.

About the role:

This opportunity isPermanent Full time, Monday to Friday during business hours (0830 - 1635 - flexible)

Supporting our Home Care program based out of our Echuca Home Care Office, the Community Registered Nurse services Uniting AgeWell clients in Mildura and the surrounding region.

  • In-home clinical assessments, reviews and care needs for clients in the East and Outer Eastern suburbs
  • Provide clinical advice and support to clients and provide referrals to services where more specialized support/advice may be required
  • Provide collaborative leadership, support and supervision to Care Advisors and Home Care staff
  • Work closely with local allied health professionals to facilitate responsive care in our clients homes
  • Provide health education and promotion, including social support groups and carer education
  • Promote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management and achieving clinical support goals
  • Office based position however flexibility is negotiable with organisation and client needs
About you:

This is the place for you if you have:

  • Bachelor of Nursing or equivalent qualification with current AHPRA Div 1 Nursing registration (required)
  • Minimum 3-5 years experience as a Registered Nurse in Community, Commonwealth Home Support or District Nursing (required)
  • Team player, flexible in your approach to work with a positive outlook
  • Endorsed for Continence Assessments and Prescription (highly desirable)
  • Experienced in actively visiting clients in their homes
  • Experienced with evaluating and implementing care support plans
  • Familiar with the My Aged Care portal
  • A kind and caring heart, with a passion for person-centered care and advocating for clients
  • Proficiency in decision making, critical analysis and on the spot problem solving
  • Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions
  • Own your own registered and insured vehicle
  • Thrive in a team and also appreciate the one to one interactions
