Job summary

Echuca Regional Health in Victoria is seeking two max-term, part-time Nurse Consultants to join the Community Palliative Care team. You will provide specialist palliative care support to patients and families living with life-limiting illness across the Campaspe Shire, coordinating with medical, allied health and community services.

• Current Registered Nurse with AHPRA and at least three years' post-graduate experience in a relevant field.

About the Role:

Applications are sought from suitably qualified Nurse Consultant to work in the Community Palliative Care team at Echuca Regional Health.Due to long term leave we currently have 2 max term roles available in Community Palliative Care Team. Be part of a team that provides specialist palliative care support to patients and families living with a life limiting illness across the Campaspe Shire. You will work collaboratively with healthcare providers and community services, the role focuses on improving quality of life through expert assessment, symptom management, care coordination and support

Key responsibilities include:

• Establishes and maintains effective therapeutic partnerships with individuals with life-limiting illnesses
• Work collaboratively with medical and allied health professionals to ensure cotemporary, evidence based palliative support is provided
• Undertake detailed assessment and care planning
• Offer advocacy and ensure that the patient’s wishes and preferences are honoured
• The Community Palliative Care Nurse will coordinate and participate in community bereavement activities
• Participate in departmental and organizational meetings as required

Qualifications/experience required:

• Current Registered Nurse with AHPRA with a minimum of three years post graduate experience in a relevant field and possessing or willing to work towards a post graduate qualification in Palliative Care.
• Highly developed interpersonal communication skills with the ability to share knowledge with clinicians across multiple disciplines and settings
• Working knowledge of medications commonly used in palliative care and how to adjust dosages.
• Ability to conduct thorough physical, emotional and social assessments in the home environment.
