Job summary

Echuca Regional Health in Victoria is seeking two max-term, part-time Nurse Consultants to join the Community Palliative Care team. You will provide specialist palliative care support to patients and families living with life-limiting illness across the Campaspe Shire, coordinating with medical, allied health and community services.

Qualifications

• Current Registered Nurse with AHPRA and at least three years' post-graduate experience in a relevant field.
• Willing to work towards a postgraduate qualification in Palliative Care.
• Strong interpersonal communication and collaboration across disciplines.
• Ability to conduct comprehensive assessments in home or community settings.

Responsibilities

• Establishes and maintains therapeutic partnerships with individuals with life-limiting illnesses.
• Collaborates with medical and allied health professionals to provide evidence-based palliative support.
• Undertake detailed assessment and care planning.
• Offer advocacy and ensure patient wishes and preferences are honoured.
• Coordinate and participate in community bereavement activities.
• Participate in departmental and organizational meetings as required.
