Job summary

Central Coast Local Health District is seeking a Community Nurse for the Virtual Care Team. You will combine clinical nursing with virtual care to support patients transitioning from hospital to home, promoting independence and preventing unnecessary readmissions.

The role requires RN registration with AHPRA, a passion for patient-centred care, and strong communication. Travel across the Central Coast and use of virtual care tech are involved.

Qualifications

• You hold a current Authority to Practice as an RN with AHPRA.
• You have a passion for patient-centred care and improving health outcomes via virtual care.
• You are a skilled communicator with patients, families and providers.
• You hold a current Australian Driver's Licence and can travel across the Central Coast.
• You can use virtual technologies and evidence-based practices to support patients.

Responsibilities

• Assess patient needs and develop individualised care plans.
• Support safe and timely transitions from hospital to home.
• Identify opportunities for early intervention and preventative care.
• Help reduce avoidable hospital presentations and admissions.
• Promote recovery, independence, health and wellbeing.
• Provide continuity of care through collaboration and effective communication.
