Job summary

Southern NSW Local Health District is seeking a Registered Nurse to join the North Sydney Centre of Northern Sydney Home Nursing in a Permanent Part Time role. You will provide quality home nursing care across the lower North Shore, working 32 hours per week with remuneration ranging from $41.23 to $57.88 per hour.

The role requires wound care, catheter care, continence management, and palliative care within a collaborative multidisciplinary team, with a focus on patient-centred, holistic care

Qualifications

• Registered Nurse – Division 1 with AHPRA and NSW Drivers Licence.
• Ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.
• Demonstrated ability to work well within a multidisciplinary team, with emphasis on service delivery, patient safety, quality of care and team cohesion.
• Sound experience using electronic medical record systems and office applications.

Responsibilities

• Provide quality home nursing care to patients in their homes within a defined geographic area.
• Deliver wound care, compression therapy, medications administration, catheter care and continence management; palliative care as required.
• Work as Primary Nurse or relieving nurse as allocated by the Nursing Unit Manager.
• Coordinate care with multidisciplinary team to ensure patient-centred outcomes.

Where you'll be working

You will be working with the Team at North Sydney Centre going into community settings on the lower North Shore.

What you'll be doing

The NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care . A position exists for a Registered Nurse to provide quality care to patients requiring community nursing care in their home, using an individualised, holistic and family-centred approach. The registered nurse works as either a Primary Nurse to patients within a defined geographic area or as a relieving nurse as allocated by the Nursing Unit Manager or as delegated. Nursing care and interventions includes wound care, compression therapy, medication administration, Catheter care and management, changing SPC and IDC, continence care and management, palliative care etc.
