Job summary

Ballarat Community Health is seeking a part-time Community Nurse (Enrolled Nurse) for the Allied Health / CN&SCP team in Ballarat. The role supports older people to stay healthy and independent at home and in clinic settings, delivering person-centred nursing care.

Working under the direction of a Registered Nurse, you will undertake care planning, clinical assessments and help older people with daily living activities. Registration with AHPRA as an EN is required.

Qualifications

• Must hold a Certificate IV or Diploma of Nursing.
• Registered with AHPRA as an enrolled nurse.
• Experience supporting older people in community/home-based care.

Responsibilities

• Provide nursing care to help older people maintain health, wellbeing and independence across home and clinic settings.
• Undertake care planning and clinical assessments; identify changing needs.
• Support daily living activities within capacity and escalate clinical concerns when required.
• Collaborate with families, carers and the multidisciplinary team to coordinate care.
• Maintain accurate clinical documentation and contribute to service improvements.

As a Community Nurse (EN), you will provide person-centred nursing care to older people across home and clinic settings. You will build genuine relationships, understand what matters to each person, support their choices and goals, identify changing needs and help connect them with the right care and services.

Is this you?

You:

• care deeply about supporting older people to maintain their health, wellbeing and independence
• take the time to listen, understand what matters to older people, and support them to participate in decisions about their care
• enjoy building relationships with older people and working in their homes
• communicate calmly and respectfully with older people, families, carers and colleagues
• hold a Certificate IV or Diploma of Nursing
• are currently registered with AHPRA as an enrolled nurse
• have experience supporting older people in a community or home-based environment

Your new role

You will provide nursing care to help older people maintain their health, wellbeing and independence across home and clinic settings.

This role focuses on care planning, clinical assessments, identifying changing needs and supporting older people to undertake daily living activities within their capacity.

You will work under the direction of a Registered Nurse, seeking guidance and escalating clinical concerns or changes in client conditions as required.

You will work closely with older people, their families, carers and the broader multidisciplinary team to support coordinated care.

You will also maintain accurate clinical documentation and contribute to the continuous improvement of community nursing services.
