Job summary

Breakthru, part of the Royal Rehab Group, seeks a casual team member to join our Coonabarabran team. You’ll work in the community, supporting individuals with daily living, orientation to home, work, and social activities, with open availability outside 9 am–3 pm.

You’ll promote independence and safety in a structured setting, complete daily case notes, and engage in a range of activities. Must reside within travel distance to Coonabarabran and be comfortable with on-call shifts.

Qualifications

• Criminal record check is mandatory (essential).
• Working with children’s check is essential or willing to obtain.
• NDIS Workers screening clearance is essential or willing to obtain.

Responsibilities

• Access the community for shopping, appointments and day-to-day activities.
• Participate in activities such as cooking, art, outdoor physical activities and excursions.
• Engage in social events and provide transportation assistance where required.
• Meal preparation and other in-home supports, including personal care.
• Documentation through daily case notes and report writing.
• Promote independence, dignity and choice in a safe, structured environment.

About The Role

We’re seeking a casual team member to join our wonderful Coonabarabran team, someone who has open availability. With flexibility to adapt to the needs of the individuals we support.

Key Responsibilities Include

• Accessing the community for shopping, appointments and day-to-day activities
• Participation in a range of scheduled activities such as cooking, art, outdoor physical activities and excursions
• Engagement in social events
• Transportation assistance where required
• Meal preparation and other simple in-home supports
• Responsible for all aspects of personal care.
• Providing goal focused supports for activities, Programs, community supports.
• In home supports
• Transport and travel
• Supports related to specific health needs
• Report writing including daily case notes
• Your role will go beyond daily care - you will be instrumental in promoting independence, dignity, and choice, while maintaining a safe, structured, and therapeutic environment

Please note: Must have availability outside the hours of 9-3, must reside within travelling distance to Coonabarabran
