Breakthru, part of the Royal Rehab Group, is seeking a casual Community Disability Support Worker in Charlestown. You’ll work in the community with clients, supporting daily living, activities, and transport, including night shifts and weekend work.

You’ll be part of a friendly, values-driven team; essential checks include Working with Children’s, Criminal Record, and NDIS screening. Immediate start offered with open availability.

Qualifications

• Criminal record check (essential or willing to obtain).
• Working with children’s check (essential or willing to obtain).
• NDIS Workers screening clearance (essential or willing to obtain).

Responsabilités

• Accessing the community for shopping, appointments and day-to-day activities.
• Participation in a range of scheduled activities such as cooking, art, outdoor physical activities and excursions.
• Engagement in social events.
• Transportation assistance where required.
• Meal preparation and other simple in-home supports.
• Responsible for all aspects of personal care.
• Providing goal focused supports for activities, Programs, community supports.
• In home supports.
• Transport and travel.
• Supports related to specific health needs.
• Report writing including daily case notes.
• Promoting independence, dignity, and choice in a safe, structured, and therapeutic environment.

About the role…

We’re seeking a casual team member to join our Charlestown teamwith open availability. The role includes active night shifts, personal care, rotating rosters, short shifts, regular and occasional weekend work, driving a pool car, and having regular access to a private vehicle to transport participants.

Key responsibilities include:

• Your role will go beyond daily care – you will be instrumental in promoting independence, dignity, and choice, while maintaining a safe, structured, and therapeutic environment
