Breakthru in Charlestown, NSW, is seeking a casual team member to join our community-support team. The role includes active night shifts, personal care, rotating rosters, short shifts, regular and occasional weekend work, driving a pool car, and access to a private vehicle to transport participants.

You will help participants achieve goals in home and community settings, promoting independence, dignity, and choice within a safe, structured, therapeutic environment.

• Criminal record check (essential or willing to obtain)
• Working with children’s check (essential or willing to obtain)
• NDIS Workers screening clearance (essential or willing to obtain)

Responsabilidades

• Accessing the community for shopping, appointments and day-to-day activities
• Participation in a range of scheduled activities such as cooking, art, outdoor physical activities and excursions
• Engagement in social events
• Transportation assistance where required
• Meal preparation and other simple in-home supports
• Responsible for all aspects of personal care.
• Providing goal focused supports for activities, Programs, community supports.
• In home supports
• Transport and travelSupports related to specific health needs
• Report writing including daily case notes
• Your role will go beyond daily care - you will be instrumental in promoting independence, dignity, and choice, while maintaining a safe, structured, and therapeutic environment
