Evolve Talent is hiring a Community Clinical Manager in Australia to energise and develop a clinical team delivering person-centred home-care across the region.

You will advise clients on clinical needs, conduct in-home assessments, and ensure care plans meet lifestyle goals, funding requirements, and quality standards while promoting compliance and continuous improvement.

Qualifications

• Requires AHPRA-registered Registered Nurse with leadership experience.
• Experience achieving accreditation standards.
• Understanding of HCP, NDIS & DVA funding streams; residential aged care helpful.

About the Role:

As a Community Clinical Manager, where you'll energise, guide, and develop your team to deliver outstanding, person-centred care across your region. You'll play a key role in shaping a connected, positive, and supportive community for every client you serve.

Bring your excellent communication and leadership skills to the forefront as you advise clients on their clinical needs, complete in-home assessments, and ensure care plans meet lifestyle goals, package requirements, and organisational standards - all while championing compliance and quality.

With your background in training and developing clinical teams, you'll lead with collaboration, identify growth opportunities, and empower your staff to excel and continuously improve.

Main Duties:

• Provide clinical guidance to participants & care staff
• Undertake assessments & case conferencing in line with standards & funding streams
• Accurate completion, review & updating of relevant documentation
• Motivation, training & development of clinical team members
• Ensure compliance & quality indicators are met

Skills & Requirements:

• AHPRA registration as a Registered Nurse with leadership experience & full Australian working rights
• Up to date working knowledge & experience achieving accreditation standards
• Strong understanding of HCP, NDIS & DVA funding streams - those with residential aged care experience will also be conidered
