Melton City Council is seeking Community Care Workers to join the Client Connections team within the Community Care & Active Living department. You will deliver high-quality, person-centred support under the CHSP and HACC-PYP programs, including domestic assistance, personal care, social support, and transport where needed.

• Experience working with aged or elderly community members, and with people with chronic or long-term illnesses.
• Genuine commitment to person-centred care and supporting older people and younger people with disabilities.
• Physically fit and able to perform everyday tasks such as vacuuming, lifting, and practical household support.
• Knowledge of CHSP and HACC-PYP principles, including independence, wellbeing, social inclusion, and carer support.
• Strong communication skills and the ability to communicate with clients, carers and their representatives in a positive and supportive manner and gain trust.
• Strong understanding of commitment to duty of care.
• Current valid Victorian Driver’s Licence, appropriately registered and comprehensively insured vehicle for transporting clients.
• Must hold or be able to obtain an Employee Working with Children Check.

Responsabilidades

• Provide community care support including domestic assistance, personal care, flexible respite, individual social support and other services to eligible CHSP PYP and HACC clients.
• Support clients to maintain and develop independent living skills and routines.
• Monitor the wellbeing of clients and report concerns to the Supervisor.
• Foster positive relationships with clients, respecting cultural values and individual choices.
