Job summary

BlueCare, part of the UnitingCare family, is seeking a Care Partner to deliver person-centred, high-quality community care across Bowen and surrounding areas. The role involves planning, coordinating services, and traveling between locations to support clients in the community.

You will work with clinical teams to achieve optimal outcomes, developing care plans and navigating funding streams while maintaining autonomy on a mobile schedule.

Qualifications

• Certificate IV in a Health or Community Service discipline is essential.
• Current Australian Driver’s Licence is essential.
• Roadworthy and insured vehicle for work is essential.
• Knowledge of client-centred, community-based care delivery.
• Able to liaise with clinical and care teams to provide coordinated care.

Responsibilities

• Develop and review care plans with client and care team.
• Manage budgets and coordinate multidisciplinary care activities.
• Liaise with clinical teams to respond to changing needs.
• Monitor and evaluate care plans and outcomes.
• Assist clients in navigating the care system and linking to resources.
