BlueCare, part of UnitingCare, is seeking a Clinical Care Partner to oversee and coordinate complex clinical and care needs in community settings. You will develop and review care plans with clients and multidisciplinary teams, manage budgets, and liaise with clinical teams to respond to changing needs, ensuring high quality, person-centered care.

Qualifications

• Registered Nurse or Allied Health professional with current AHPRA registration.
• Strong understanding of client-centred, community-based care delivery.
• Experience with community funding streams and programs.

• Manage budgets and facilitate multidisciplinary activities.
• Liaise with clinical teams to monitor changing needs.
