Job summary

UnitingCare Queensland's BlueCare division is seeking a Care Partner to deliver person-centred, high-quality community care across Bowen, Proserpine and surrounding areas. You will work with multidisciplinary teams to plan and coordinate services, while spending significant time on the road visiting clients.

You will develop and review care plans, manage budgets, and help clients navigate the care system, ensuring outcomes meet funding requirements and client needs in a supportive, compassionate

Qualifications

• Certificate IV in Health or Community Service discipline (Essential).
• Current Australian Driver’s Licence (Essential).
• Roadworthy vehicle to use for work (Essential).
• Knowledge of client-centred, community-based care delivery.
• Ability to liaise with clinical and care teams to coordinate care.
• Experience with community funding streams and programs.
• Excellent communication and clinical problem-solving skills.
• Time management and autonomous work ability.
• Willingness to satisfy probity checks.

Responsibilities

• Develop and review care plans with client and care team.
