Job summary

BlueCare, part of UnitingCare, seeks a Care Partner to deliver person-centred care, oversee care planning, and coordinate services in community settings. You will work closely with clinical teams to ensure optimal outcomes for clients funded through aged care services.

The role requires driving to client sites, travel across Redland Bay to surrounding areas, and strong communication skills to support diverse client needs. A Certificate IV and a current driver’s licence are essential.

Qualifications

• Certificate IV in Health or Community Service (Essential)
• Current Australian Driver’s Licence (Essential)
• Vehicle for work (inspected/insured) (Essential)
• Strong client-centred care understanding and delivery

Responsibilities

• Develop and review care plans with the client and care team
• Manage budgets and coordinate multidisciplinary activities
• Liaise with clinical teams to respond to changing client needs
• Monitor and evaluate care plans, services and outcomes
• Assist clients to navigate the care system and resources
