Job summary

BlueCare is seeking a Care Partner to provide person-centred, high-quality care in the community across New Farm, Hamilton, Kedron, McDowall and surrounding areas. You will oversee care planning, coordinate services with clinical teams, and support clients to access resources and navigate the care system.

This permanent full-time role involves significant road-time delivering community-based care. Applicants should have a Certificate IV in Health or Community Service, a current driver’s licence,

Qualifications

• Certificate IV in a Health or Community Service discipline (Essential)
• Current Australian Driver’s Licence (Essential)
• Roadworthy & insured vehicle for work (Essential)
• Demonstrated understanding of client-centred, community-based care services delivery
• Ability to liaise with clinical and care teams to provide coordinated care

Responsibilities

• Develop and review care plans in collaboration with the client and multidisciplinary care team
• Manage budgets and facilitate multidisciplinary care activities
• Liaise with clinical teams to monitor and respond to changing needs of complex community clients
• Continuously monitor, review and evaluate care plans, services and outcomes
• Assist clients in navigating the care system and linking them to resources as required
