Job summary

Northside Community Service in North Canberra is seeking a compassionate Registered Nurse to join the Aged & Community Care team, delivering clinical services under the Support at Home program and supporting older people to maintain independence.

You will work autonomously across the community, collaborating with clients, families, GPs and care teams to achieve positive health outcomes. Northside Community Service values social justice, diversity and inclusivity.

Qualifications

• Current unrestricted AHPRA RN registration.
• Experience in aged care or community nursing.
• Strong assessment, clinical decision-making and problem-solving skills.
• Excellent communication and relationship-building abilities.
• Commitment to person-centred care, client advocacy and cultural safety.
• Knowledge of aged care legislation, quality standards and community care models.
• Current CPR certification.
• Current Australian Driver's Licence and access to reliable transport.

Responsibilities

• Conduct comprehensive clinical and risk assessments.
• Develop, implement and review individualised care plans.
• Deliver nursing care including wound care, medication management and chronic disease management.
• Coordinate services with healthcare providers and community stakeholders.
• Support and educate clients, families and carers to improve health outcomes.
• Provide clinical guidance to care workers and care partners.
