Job summary

Calvary Health Care Kogarah is seeking a Community Aged Care Manager to lead the Commonwealth funded community aged care services. This full-time position, based at Kogarah Hospital, requires strategic leadership, multidisciplinary team management, and delivery of high-quality care within program guidelines.

The role involves rostering, performance development, and collaboration with the Director of Allied Health to set a strategic direction in line with CHCK’s values.

Qualifications

• Lead operational management of a multidisciplinary team including rostering, leave management, supervision, performance development.
• Develop and communicate a strategic direction for Commonwealth funded community aged care aligned to Calvary Health Care Kogarah's vision and values.
• Ensure delivery of values-based healthcare in alignment with program guidelines and quality standards.

Responsibilities

• Provide effective operational management of a multidisciplinary team including rostering, leave management, supervision, performance development.
• In collaboration with the Director of Allied Health and Non-Admitted Care, develop and communicate a strategic direction for Commonwealth funded community aged care services aligned to CHCK’s vision, strategies and organisational values.
• Ensure CHCK delivers values based health care in alignment with the Transitional Aged Care Program, Commonwealth Home Support Program and any other Commonwealth funded community aged care program guidelines
