Job summary

Calvary Health Care Kogarah in NSW is seeking a Community Aged Care Manager to provide strategic leadership for Commonwealth funded community aged care programs and manage the Transitional Aged Care Program Team on a full-time basis.

You will lead a multidisciplinary team, ensure safe, high quality service delivery, align programs with CHCK’s vision and compliance requirements, and drive performance through coaching and KPI management.

Qualifications

• Relevant tertiary qualifications or equivalent experience in healthcare management.
• Experience managing community health programs (e.g., Transitional Aged Care Program) including budgets, workforce and KPIs.
• Proven ability to lead multidisciplinary teams and coach staff to achieve targets.

Responsibilities

• Provide operational management of a multidisciplinary team including rostering and performance development.
• Develop strategic direction for Commonwealth-funded community aged care services aligned to CHCK’s vision.
• Ensure CHCK delivers values-based care in alignment with program guidelines.
