Job summary

Barwon Health is seeking a Registered Pharmacist to join the McKellar Centre team in a part-time role, 9:00am to 2:30pm on weekdays. The Clinical Pharmacist Grade 1 provides clinical and operational support, focusing on sub-acute services including rehabilitation, palliative care and geriatric pharmacy.

Qualifications

• Registered Pharmacist with AHPRA and hospital/clinical pharmacy experience preferred.
• Strong pharmacotherapy knowledge and evidence-based medication management.
• Excellent communication and teamwork across multidisciplinary teams.

Responsibilities

• Deliver clinical pharmacy services across sub-acute and inpatient services, including reconciliation and discharge planning.
• Support safe and cost-effective medication use and formulary compliance.
• Collaborate with medical and nursing staff to optimise patient care and workflow.
• Contribute to education, quality improvement and student supervision.
