Job summary

Queensland Health on the Sunshine Coast is seeking a Pharmacist Senior ARC to deliver advanced clinical pharmacy services to older adults across acute, restorative and rehabilitation settings. The role emphasizes geriatric syndromes, polypharmacy management, medicines optimisation, deprescribing and safe transitions of care.

You will lead clinical reviews, mentor staff, and contribute to the pharmacy education program within Sunshine Coast Health. Remuneration up to $157,359 p.a.

Qualifications

• Provides advanced clinical pharmacy services to older adults with complex health needs across acute, restorative and rehabilitation stages.
• Leads medication reviews and formulates medication management plans.
• Mentors and educates junior staff and contributes to the pharmacy education program.

Responsibilities

• Document admissions including best possible medication history and clinical reviews.
• Conduct clinical medication reviews and collaboratively optimise medication management plans.
• Provide supervision, education, and mentorship to staff and support personnel.
