Job summary

IRT Dalmeny Residential Care Home is seeking an on-site pharmacist to join our multidisciplinary team. You will bring expertise in medication management and pharmacotherapy to residents, collaborating with GPs, nurses and caregivers to deliver person-centred care.

The role is part-time, 1 day per week, with focus on improving medication safety and outcomes in a supportive not-for-profit setting.

Qualifications

• Registration as a pharmacist with AHPRA.
• ACOP training credentialing requirement.
• MMR credentialed or recognition prior to 30 June 2026 for Measure participants.

Responsibilities

• Provide medication management and pharmacotherapy in residential aged care facilities.
• Collaborate with GPs, on-site nurses and facility staff to optimise resident health outcomes.
• Monitor residents’ medications and support transitions of care.
• Engage in whole facility medicine use evaluations and safety initiatives.
