Job summary

Barwon Health in Geelong, VIC, AU is seeking a Lead Aged Care On-Site Pharmacist (Grade 3) for a full-time, on-site role. You will lead pharmacy services across residential aged care facilities and care transitions, ensuring safe, appropriate, and efficient medication use in line with Barwon Health priorities.

You will drive quality use of medicines, contribute to governance and education, and broaden your practice across hospital settings while supporting continuous quality improvement for

Qualifications

• Current registration as a Pharmacist with AHPRA and significant hospital pharmacy experience.
• Credentialed for the Australian Pharmacy Council (APC)-accredited aged care on-site pharmacist training program.
• Credentialed for Medication Management Reviews (MMR).
• Excellent leadership, communication and stakeholder engagement skills, with the ability to influence organisation-wide practice.

Responsibilities

• Lead delivery of high-quality pharmacy services across Barwon Health’s aged care facilities and care transitions.
• Ensure safe, appropriate and efficient medication use in line with program objectives.
• Contribute to medicines governance, reporting and advice to the Medicines Advisory Committee.
• Support education and development of pharmacy interns and students, and undertake clinical pharmacy research.
• Broaden skills across hospital pharmacy settings and participate in governance activities.
