Job summary

Catholic Healthcare in Inner West Sydney is seeking a dedicated Community Nurse to support older adults living at home. You will provide direct clinical nursing care, assessments, wound care, and education as part of a multidisciplinary team.

This role involves travel to clients’ homes, with a permanent part-time or full-time arrangement Monday to Friday, and benefits such as tax-efficient packaging and km reimbursement.

Qualifications

• Current AHPRA registration following completion of a Bachelor of Nursing or similar.
• Minimum 3 years post graduate nursing experience.
• Experience in clinical assessments – PAS & RUDAS.
• Willingness to apply for a Police Check and NDIS clearance.
• Drivers license and a reliable vehicle.

Responsibilities

• Providing direct clinical nursing care to our clients in their homes in line with best practice nursing procedures
• The duties will be varied and will include clinical assessments, dressings, medication assessment and catheter changing
• Develop a holistic nursing care plan in collaboration with client, carer, Care Advisor and Allied health where relevant
• Provide safe, quality nursing practice to clients with specific needs and/or complex needs eg. Wound care, palliative care, education and support
• Liaise with all parties involved the care of the client to maintain holistic health care and wellbeing
• Ensure all documentation is accurate and completed in a professional and timely manner
• Working in a safe manner to protect yourself, your colleagues and clients closely following all policies and procedures

To be successful in this role you’ll need

• A kind heart and genuine passion for providing high quality care to the elderly
• Current AHPRA registration following completion of a Bachelor of Nursing or similar
• Minimum 3 years post graduate nursing experience
• Experience in clinical assessments – PAS & RUDAS
• Strong communication skills and a positive attitude
• Sound computer skills especially Microsoft Office products and ability to pick up new systems
• Drivers license and a reliable vehicle
• Willingness to apply for a Police Check and NDIS clearance
