Job summary

Respect in Wangaratta is seeking a full-time Clinical Care Partner – Home Care to deliver safe, person-centred clinical care, assessments and care plans for participants in their homes. You will work with GPs, allied health and other providers to ensure coordinated, evidence-based care aligned with aged care standards.

You will oversee restorative pathways, contribute to planning and delivery, and support ongoing professional development in a growing, not-for-profit service focused on dignity

Qualifications

• Current and unrestricted AHPRA registration (RN/EN).
• Strong clinical judgement, communication and problem-solving skills.
• A person-centred approach with commitment to participant dignity and quality of life.
• Understanding of clinical risk, escalation and safe practice.
• Ability to build effective relationships with participants, families and health providers.
• Digital and documentation skills, including electronic records.

Responsibilities

• Deliver safe, person-centred clinical care that promotes wellbeing and independence.
• Complete clinical assessments and develop individualised care and reablement plans.
• Provide clinical nursing interventions including wound care and chronic disease management.
• Monitor clinical needs and document escalations as required.
• Coordinate referrals with GPs and external health providers.
• Lead Restorative Care Pathways and support palliative care.
• Provide clinical guidance and delegation support to Care Partners.
• Maintain governance, documentation and participant advocacy.
