GD Recruitment PTY LTD in Brisbane is seeking an experienced RN to become Clinical Care Manager in community aged care, blending clinical oversight, team support and service development in a collaborative environment.

This role involves conducting client assessments, coordinating care across nursing and allied health, and managing budgets to ensure high-quality care in line with Aged Care Quality Standards; a hybrid work model is offered to support work-life balance.

Formación

• Current AHPRA registration (RN, EN or Allied Health).
• Experience in community aged care and case management.
• Understanding of Support at Home funding, care planning and budgets.
• Experience managing an individual client caseload.
• Experience supporting clients with complex care needs.
• Strong communication and relationship-building skills.
• Organised, proactive and confident using client management systems.

Clinical Care Manager - Community Aged Care

A well-established Brisbane community provider is now seeking an experienced RN to step into an exciting Clinical Care Manager role. This position offers a strong mix of clinical oversight, team support and service development within a collaborative, forward-thinking environment.

You’ll play a key role in providing high quality clinical practice and enhancing client outcomes, with a great balance of office and field work.
