Job summary

Greenwood Manor Aged Care in Melbourne is seeking a Clinical Care Coordinator to lead our clinical team and deliver safe, person-centred care. This permanent full-time role runs 07:00–15:06, Monday to Friday, with backing to advance your leadership career.

You will collaborate with GPs and allied health, oversee assessments, care plans, medication management, and compliance with Aged Care Quality Standards, while mentoring staff and driving continuous improvement in a values-driven MACG

Qualifications

• Current AHPRA registration as a Registered Nurse.
• Experience in aged care with leadership experience or a desire to move into leadership.
• Knowledge of Aged Care Quality Standards and accreditation requirements.
• Clear written and verbal communication with a collaborative approach.
• Experience using clinical software and maintaining accurate documentation.
• Current Employment Police Check and Australian Working rights.

Responsibilities

• Provide clinical leadership and day-to-day guidance to nursing and care staff.
• Oversee resident assessments, care planning and evaluations to ensure person-centred care.
• Coordinate medication management, clinical incident follow-up and infection control.
• Liaise with GPs and allied health professionals, residents and families.
• Support compliance with Aged Care Quality Standards and accreditation.
• Utilise best-practice software to streamline processes and documentation.
• Mentor staff, assist rostering and workforce planning.
• Drive continuous improvement and quality outcomes for residents.
• Participate in community outreach and health promotion initiatives.

About the Role:

We have a permanent full-time opportunity for a Clinical Care Coordinator at Greenwood Manor Aged Care. The hours are from 0700 to 1506 Monday to Friday.

Lead and support our clinical team to deliver safe, high quality, person-centered care. Whether you’re an experienced clinical leader or ready to step up and grow your leadership skills, you’ll have the backing to take charge of your career and make a real difference to residents and colleagues.

Key Responsibilities:

• Provide clinical leadership and day-to-day guidance to the nursing and care team
• Oversee resident assessments, care planning and evaluations, so care is person-centered and up to date
• Coordinate medication management, clinical incident follow-up and infection control practices
• Liaise with GPs, allied health professionals, residents and families about care needs
• Support compliance with the Aged Care Quality Standards and accreditation requirements
• Use our best practice software to streamline processes and keep documentation accurate
• Mentor, support and develop staff, and contribute to rostering and workforce needs
• Drive continuous improvement and quality outcomes for residents
• Take part in community outreach and health promotion initiatives

Experience and Qualifications:

• Current AHPRA registration as a Registered Nurse
• Experience in aged care, with clinical leadership experience or a strong desire to step into leadership
• Knowledge of the Aged Care Quality Standards and accreditation requirements
• Strong clinical assessment, decision-making and problem-solving skills
• Clear written and verbal communication skills, with a collaborative approach
• Confidence using clinical software and keeping accurate documentation
• Current Employment Police Check (required)
• Australian Working rights (required)
