Greenwood Manor Aged Care, operated by Medical & Aged Care Group (MACG), seeks a Clinical Care Coordinator to lead the clinical team in delivering safe, person-centered care. Join a close-knit team at a 52-bed home in Dingley Village with a focus on quality improvement and resident well-being.

Role involves leading assessments, care planning, medication management, and liaison with GPs and families. Monday–Friday 07:00–15:06, on-site with ongoing training and development opportunities.

Formación

• Current AHPRA registration as a Registered Nurse.
• Experience in aged care with clinical leadership experience or a strong desire to step into leadership.
• Knowledge of the Aged Care Quality Standards and accreditation requirements.
• Strong clinical assessment, decision-making and problem-solving skills.
• Clear written and verbal communication skills, with a collaborative approach.
• Confidence using clinical software and keeping accurate documentation.
• Current Employment Police Check (required).
• Australian Working rights (required).

Responsabilidades

• Provide clinical leadership and day-to-day guidance to the nursing and care team.
• Oversee resident assessments, care planning and evaluations, so care is person-centered and up to date.
• Coordinate medication management, clinical incident follow-up and infection control practices.
• Liaise with GPs, allied health professionals, residents and families about care needs.
• Support compliance with the Aged Care Quality Standards and accreditation requirements.
• Use our best practice software to streamline processes and keep documentation accurate.
• Mentor, support and develop staff, and contribute to rostering and workforce needs.
• Drive continuous improvement and quality outcomes for residents.
• Take part in community outreach and health promotion initiatives.
