Job summary

Respect Riverside Gardens in Nambucca Heads, NSW, is seeking an experienced Registered Nurse to step into the role of Clinical Care Coordinator. You’ll provide clinical leadership, mentor nursing staff and ensure safe, person-centred care for residents.

Working with the Care Manager and multidisciplinary teams, you’ll lead admissions, care planning and clinical governance while maintaining hands-on nursing support. This is a role for driven leaders committed to quality aged care.

Qualifications

• Bachelor of Nursing and current AHPRA registration.
• Minimum 2 years’ nursing experience in residential aged care.
• Experience in clinical leadership or care coordination highly regarded.
• Strong clinical assessment, care planning and documentation skills.

Responsibilities

• Coordinate admissions, assessments and develop individualised care plans.
• Provide clinical leadership and support to nursing and care teams.
• Mentor staff to promote high-quality clinical practice.
• Provide hands-on nursing care, including medication management and wound care.
• Collaborate with residents, families, GPs and allied health professionals.
• Maintain high standards of clinical documentation and care planning.
• Support infection prevention and compliance with standards.
• Identify opportunities for continuous improvement and contribute to quality initiatives.
• Support audits, accreditation and regulatory requirements.

About the role

As Clinical Care Coordinator, you’ll play a key role in ensuring our residents receive safe, high-quality and personalised care.

Working closely with the Care Manager, nursing team, care staff, residents, families and external healthcare professionals, you’ll provide clinical leadership while remaining connected to the day-to-day care of our residents.

This is a hands‑on leadership role where you’ll have the opportunity to mentor and support nursing staff, strengthen clinical practice, contribute to care planning and drive positive resident outcomes.

What you’ll be doing

No two days are the same. You’ll have the opportunity to:

• Coordinate admissions, complete comprehensive assessments and develop individualised care plans.

• Provide clinical leadership, guidance and support to nursing and care teams.

• Mentor and develop staff to promote high-quality clinical practice.

• Provide hands-on nursing care, including medication management, wound care and clinical monitoring.

• Work closely with residents, families, GPs, allied health professionals and other healthcare providers.

• Maintain high standards of clinical documentation and care planning.

• Support infection prevention and control practices and ensure compliance with relevant standards and legislation.

• Identify opportunities for continuous improvement and contribute to quality improvement initiatives.

• Support the team through audits, accreditation and regulatory requirements.

About you

We’re looking for an experienced Registered Nurse who is ready to make an impact and take their career to the next level.

You’ll bring:

• Bachelor of Nursing and current AHPRA registration.

• Minimum 2 years’ nursing experience in residential aged care.

• Experience in a clinical leadership or care coordination role will be highly regarded.

• Strong clinical assessment, care planning and documentation skills.

• The ability to lead, mentor and support a multidisciplinary team.

• Excellent communication and relationship‑building skills.

• A genuine commitment to person‑centred care and achieving positive outcomes for residents.

• Confidence using clinical software and Microsoft applications.

• An understanding of aged care accreditation, quality standards and regulatory requirements.

• Eligibility to meet Aged Care Worker Screening requirements.
