Job summary

My Guardian in Sydney is seeking a Clinical Care Coordinator to lead the clinical heart of our growing home care service. You will conduct home assessments, design individualised care plans, and oversee wound care, medications and allied health coordination.

As an AHPRA-registered nurse, you’ll mentor staff, ensure quality, safety, and NDIA compliance, and build strong relationships with clients, families and hospitals. A supportive team and development opportunities await.

Qualifications

• Current AHPRA Registered/Enrolled Nurse Registration is essential.
• Strong foundational nursing skills with a quality-first mindset.
• Exceptional verbal and written communication skills, building relationships daily.

Responsibilities

• Lead care planning with home assessments and personalised care plans.
• Mentor and supervise care staff with clinical guidance.
• Hands-on clinical care including wound care and medication administration.
• Coordinate continuity between clients, families, hospitals and allied health professionals.

The Role

As a Clinical Care Coordinator, you will be the clinical heart of our expanding service, ensuring high-quality, compliant care across our growing community.

• Lead Care Planning: Conduct home assessments and design individualised care plans that empower independence.

• Hands-On Clinical Care: Manage wound care, medication administration, and specialised treatments.

• Advocate: Support NDIS and My Aged Care compliance to ensure every client hits their personal goals.
