Nepean Blue Mountains Local Health District (NBMLHD) is seeking a compassionate Clinical Aged Care Assessor to join our team at Lemongrove Campus. You will assess care needs for older people across hospital and community settings, develop person-centred care plans, and facilitate access to aged care programs that promote independence and wellbeing.

As part of a multidisciplinary team, you will work across ACA sites, use My Aged Care knowledge, and support families and care providers.

Formación

• Relevant tertiary qualifications in Nursing/ Occupational Therapy/ Physiotherapy with AHPRA registration.
• Alternatively for Social Worker/Dietitian/Speech Pathologist, a relevant degree with eligibility for professional body membership.
• Proven high standard of written and verbal communication.

Responsabilidades

• Conduct holistic assessments of older people in hospital or community settings.
• Develop person-centred care recommendations and tailored care plans.
• Collaborate with multidisciplinary team to ensure timely aged care access.
• Utilise My Aged Care knowledge to support routine and complex presentations.
