Job summary

The Clinical Assessor role with Central Coast Local Health District (CCLHD) sits within NSW Health and focuses on comprehensive aged care assessments using the Integrated Assessment Tool (IAT). Qualifications

• Qualifications in accordance with Schedule C of NSW Health Service Health Professionals (State) Award and current registration with AHPRA.
• Minimum of one year demonstrated experience delivering aged care services (geriatric evaluation, rehabilitation, palliative care, community nursing) to older people, including those living with dementia.

Responsibilities

• Undertake comprehensive aged care assessments using the Integrated Assessment Tool (IAT).
• Formulate person-centred support plans and recommendations to access Commonwealth-funded aged care services.
• Conduct assessments across hospital, community settings and residential facilities to determine eligibility for programs like CHSP, Support at Home and Residential Aged Care.
• Collaborate with older people, carers, health professionals and service providers to ensure timely access to appropriate care.
