Job summary

Blue Marble Health Services is seeking a compassionate Client Coordinator to deliver client-centred support for SIL and community participants on the Gold Coast. You will assess needs, develop tailored support plans, and connect people with appropriate services to enhance wellbeing and independence.

The role requires organisational skills, risk assessment, culturally sensitive practice, and the ability to navigate complex systems.

Qualifications

• Certificate IV in Human Services or related field.
• Experience in case management or service coordination for vulnerable populations.
• Ability to conduct holistic assessments across health, housing, finances, and social well-being.
• Multilingual skills are highly regarded.

Responsibilities

• Conduct comprehensive client assessments to determine needs and risks.
• Collaborate with clients to develop, implement, and regularly review personalised case or support plans
• Provide coordination for SIL and community participants.
• Support clients in navigating complex service systems and accessing essential resources
• Undertake risk assessments and develop safety plans to promote client wellbeing
• Maintain accurate, timely, and confidential case records in line with organisational and compliance requirements
• Build and maintain strong, respectful relationships with clients, families, and stakeholders
• Apply strengths-based, culturally sensitive, and trauma-informed approaches in all client interactions
• Assist clients in areas such as housing, financial stability, health access, education, and employment pathways
