Job summary

Ausdocs Holdings Group is seeking a Chronic Condition Management Nurse for our CQ Medicentre South practice in Rockhampton. The role focuses on proactive care planning, education and ongoing management of patients with chronic health conditions, within an experienced clinical team.

You will coordinate management plans, conduct health assessments, educate patients and work closely with GPs and allied health staff to improve long-term health outcomes.

Qualifications

• Current AHPRA registration as a Registered or Enrolled Nurse.
• Current CPR certification.
• Experience in General Practice nursing.
• Familiarity with Chronic Condition Management concepts.

Responsibilities

• Coordinate Chronic Condition Management plans and care reviews.
• Conduct Health Assessments and patient education sessions.
• Recall and reminder management; follow-up and documentation.
• Collaborate with GPs and allied health providers; monitor outcomes.

About the Role

This position has a strong focus on Chronic Condition Management and preventative healthcare initiatives.

Responsibilities include:

• Coordinating Chronic Condition Management plans
• Conducting Care Plan Reviews
• Completing Health Assessments
• Patient education and health coaching
• Recall and reminder management
• Working collaboratively with GPs and allied health providers
• Monitoring patient outcomes and follow‑up activities
• Maintaining accurate clinical documentation

About You

We are seeking someone who is passionate about patient‑centred care and enjoys helping patients achieve better long‑term health outcomes.

You will have:

• Current AHPRA registration as a Registered or Enrolled Nurse
• Current CPR certification
• Experience in General Practice nursing
• Demonstrated experience with or the ability to learn: Chronic Condition Management Plans, Care Plan Reviews, Health Assessments, Patient follow‑up programs.
• Excellent organisational and communication skills
• Full Australian working rights
• Experience using Best Practice software will be highly regarded.
