Job summary

CQ Medicentre South in Rockhampton is seeking an experienced Chronic Condition Management Nurse to join our established practice.

You'll focus on proactive care planning, patient education and ongoing management for patients with chronic conditions, working with a skilled team of GPs, nurses and administration staff to improve health outcomes.

Qualifications

• Current nursing registration with AHPRA is required.
• Experience in General Practice nursing is preferred.
• Experience with chronic condition management is desirable.
• Excellent organisational and communication skills.
• Full Australian working rights.

Responsibilities

• Coordinating Chronic Condition Management plans
• Conducting Care Plan Reviews
• Completing Health Assessments
• Patient education and health coaching
• Recall and reminder management
• Working collaboratively with GPs and allied health providers

About the Role

This position has a strong focus on Chronic Condition Management and preventative healthcare initiatives.

Responsibilities include:

• Coordinating Chronic Condition Management plans
• Conducting Care Plan Reviews
• Completing Health Assessments
• Patient education and health coaching
• Recall and reminder management
• Working collaboratively with GPs and allied health providers
• Monitoring patient outcomes and follow-up activities

About You

We are seeking someone who is passionate about patient-centred care and enjoys helping patients achieve better long-term health outcomes.

You will have:

• Current AHPRA registration as a Registered or Enrolled Nurse
• Current CPR certification
• Experience in General Practice nursing
• Demonstrated experience with or the ability to learn:Chronic Condition Management Plans,Care Plan Reviews,Health Assessments,Patient follow-up programs.
• Excellent organisational and communication skills
• Full Australian working rights
• Experience using Best Practice software will be highly regarded.
