Job summary

ForHealth Medical Centres is seeking a Part-Time Chronic Disease Management Nurse for our Leichhardt Medical Centre. You will work with GPs and patients to prevent and manage chronic conditions, empowering patients to improve long-term health outcomes.

You will coordinate GPMPs/TCAs, conduct health assessments, educate patients, and coordinate referrals with a multidisciplinary team to enhance preventative care and reduce hospital admissions.

Qualifications

• Current AHPRA registration as EN or RN.
• Experience in Chronic Disease Management within a general practice setting.
• Experience completing health assessments is highly regarded.
• Excellent organisational and time management skills.
• Strong communication and interpersonal skills with a patient-focused approach.
• Genuine passion for preventative healthcare and improving patient outcomes.

Responsibilities

• Prepare and coordinate GPMPs and TCAs.
• Conduct health assessments and identify patients who may benefit from chronic disease management.
• Support and educate patients to better understand and manage their health conditions.
• Coach patients on sustainable health behaviour changes.
• Coordinate referrals and ongoing care with multidisciplinary healthcare providers.
• Partner with GPs to promote preventative healthcare initiatives across the practice.
• Increase patient engagement in chronic disease management programs.
• Ensure care plans are reviewed and updated in line with clinical requirements.
• Complete all administrative tasks associated with CDM appointments accurately and efficiently.

About the Role

We're looking for an experienced Part-Time Chronic Disease Management (CDM) Nurse to join our Leichhardt Medical Centre. In this rewarding role, you'll work closely with GPs and patients to support the prevention and management of chronic conditions. You'll play a vital role in empowering patients to take control of their health, improving long-term outcomes, and reducing avoidable hospital admissions.

Key Responsibilities

• Prepare and coordinate GP Management Plans (GPMPs) and Team Care Arrangements (TCAs).
• Conduct health assessments and identify patients who may benefit from chronic disease management.
• Support and educate patients to better understand and manage their health conditions.
• Coach patients on sustainable health behaviour changes.
• Coordinate referrals and ongoing care with multidisciplinary healthcare providers.
• Partner with GPs to promote preventative healthcare initiatives across the practice.
• Increase patient engagement in chronic disease management programs.

About You

• Current AHPRA registration as an Enrolled Nurse (EN) or Registered Nurse (RN).
• Demonstrated experience in Chronic Disease Management within a general practice setting.
• Experience completing health assessments (highly regarded).
• Excellent organisational and time management skills.
• Strong communication and interpersonal skills with a patient-focused approach.
• A genuine passion for preventative healthcare and improving patient outcomes.
