Qualifications

• Criminal record check required or willingness to obtain.

Responsibilities

• Access the community for shopping, appointments, and day-to-day activities.
• Participate in activities such as cooking, art, outdoor physical activities and excursions.
• Engage in social events.
• Provide transportation assistance where required.
• Meal preparation and in-home supports.
• Responsible for all aspects of personal care.
• Provide goal-focused supports for activities, programs, community supports.
• In-home supports and transport.
• Supports related to health needs.
• Report writing including daily case notes.
• Promote independence, dignity and choice in a safe, structured environment.
