Job summary

The St Vincent de Paul Society NSW is seeking a dedicated Case Worker to provide vital support to people experiencing homelessness in Nowra. You will deliver outreach, tenancy support, and crisis case management, helping clients stabilise and secure accommodation across diverse settings.

Qualifications

  • Experience delivering support to marginalised and vulnerable people.
  • Knowledge of homelessness issues including family breakdown, mental illness and addictions.
  • Ability to build and maintain relationships with local community services and government agencies.
  • Excellent written and verbal communication; strong computer skills.

Responsibilities

  • Engage with persons sleeping rough or at risk of homelessness.
  • Undertake home visits to clients in various accommodations.
  • Refer clients to external services and life skills programs.
  • Develop and monitor case plans and provide ongoing support.
  • Maintain client records (electronic and handwritten).
  • Meet service targets as required.

Your Role

Vital and diverse role providing a range of Case Work support to people experiencing homelessness. The support you provide will be crucial in helping people stabilise and sustain their accommodation and ultimately transition away from homelessness. This is a highly rewarding and diverse role with a mixture of outreach, tenancy and crisis case management support.


Your Responsibility


  • Engage with persons sleeping rough and/or those people in danger of becoming homeless.

  • Undertake home visits with clients located in temporary, transitional or permanent accommodation.

  • Refer clients to appropriate external services and link to life skill and educational programs

  • Create collaborative partnerships with other key stakeholders and services.

  • Offer client support at all times with a respectful, non-judgmental and inclusive attitude and manner, upholding and respecting the rights of clients at all times while maintaining client confidentiality.

  • Develop case plans with the clients, provide ongoing support to achieve the case plan and monitor the case plan.

  • Attend to the primary needs of clients and maintain client records/case notes, both electronic and handwritten.

  • Meeting service targets as required by the manager, Society and/or external agencies.


You Will Need


  • Previous experience providing support to marginalised and vulnerable people support to homeless persons

  • Demonstrated knowledge of issues relating to homelessness including but not limited to family breakdown, mental illness, drug, alcohol and gambling addictions.

  • Demonstrated awareness of appropriate local community support services for homeless clients, including government agencies and NGO's, and the capacity to build and maintain strong relationships with such service providers.

  • Excellent written/oral communication skills and sound computer skills.

  • NSW driver licence (unencumbered)
