Job summary

Interrelate is a not-for-profit organisation with a long history of delivering relationship support. We seek a part-time, fixed-term case manager to engage with families, conduct assessments, and coordinate referrals across Interrelate and external services.

You will work to assess risks, help develop goals and service plans, and collaborate with a multi-disciplinary team to achieve positive outcomes for children and families.

Qualifications

• Bachelor's degree in social work, community welfare, counselling, psychology or related discipline.
• Graduate Diploma in counselling, community service or equivalent.
• Experience in case management with families and managing high parental conflict.
• Experience in therapeutic work with families and children; knowledge of child development and family violence.
• Ability to liaise with the community and build local networks including family law specialists and magistrates.
• Ability to work collaboratively in a multi-disciplinary team.
• Excellent written and verbal communication skills.

• Make appropriate internal and external referrals.

• Conduct regular case management meetings and external meetings with other services or agencies.

ABOUT THE ROLE:

Providing coordinated case management to clients through the process of engagement, screening, needs assessment and referring them to appropriate services within Interrelate and externally as required.

Key Duties will include:

• assess eligibility and suitability for services
• assess the person’s / family’s needs
• assess risks (including suicide risk) and develop action plans to mitigate risks
• assist clients / families to develop their own goals and case/service plan
• make appropriate internal and external referrals
• refer to other services including scheduling people into their initial appointmentwith Interrelate practitioners / services

Monitor progress and the changing needs of families while they are waiting for and receiving Interrelate services

Provide case management and/or brief interventions to a caseload of clients

Conduct regular case management meetings as required including attending and facilitating external meetings with other services or agencies

Employment Type:

Fixed term Part-time contract until 30 June 2027

Hours of Work:

67.5 hours per fortnight

Days of Work:

Monday, Tuesday, Wednesday & Friday, possible evening work on Monday's (12pm-8pm)

Hourly Rate:

In line with a SCHADS Award level 5 position, $57.72 per hour, plus superannuation

ROLE REQUIREMENTS:

• Bachelor's degree in social work, community welfare, counselling, psychology or related discipline; or a Graduate Diploma in counselling, community service or equivalent in addition to a bachelor's degree that is not related
• Demonstrated experience in case management with families along with the skills in managing high parental conflict whilst upholding the best interests of the child
• Demonstrated experience in therapeutic work with families and children; along with knowledge of child development, child protection and family violence issues
• Ability to liaise with the community and build local networks including family law specialist and magistrates along with experience in dealing with the Family Law System
• Ability to work collaboratively with a multi-disciplinary team to ensure positive outcomes
• Excellent written and verbal communication skills
