Case Manager - Mental Health Practitioner - Pathways to Success Program

2b Inclusive is looking for an experienced Social Worker to join our growing Pathways to Success Program, supporting people living with disability who experience complex mental health, psychosocial and social support needs.

This is a hands-on case management role for someone who enjoys working with complexity, building relationships and helping people navigate systems that can often be difficult to access. Applicants will preferably live in the northern or western Adelaide metropolitan area.

You will work alongside participants to identify what is getting in the way, connect them with the right services, advocate when needed and continue following things through rather than simply making a referral.

What you'll be doing

Provide intensive, person-centred case management.

Support people experiencing complex mental health, psychosocial and disability-related needs.

Complete assessments and develop practical case management goals.

Help participants access mental health, health, housing, disability and community services.

Advocate with and for participants where barriers to services exist.

Coordinate with GPs, mental health teams, government agencies, community services and other professionals.

Provide assertive engagement and outreach for people who may find it difficult to engage with services.

Attend case conferences and multidisciplinary meetings.

Maintain clear, professional and timely case notes and documentation.

Travel within metropolitan Adelaide and regional South Australia. Regular regional travel is an important part of this role, including travel to areas such as the Riverland, depending on participant and program needs. Applicants must be comfortable travelling to regional locations, including undertaking regular day trips, and hold a current driver's licence.

About you

You will need:

A Bachelor or Master of Social Work.

Experience working professionally as a Social Worker.

Experience supporting people with complex mental health, psychosocial or social needs.

Strong case management, advocacy and service-navigation skills.

Confidence working with complex situations and multiple service systems.

Excellent communication and relationship-building skills.

The ability to work independently while knowing when to seek support or supervision.

A practical, persistent and genuinely person-centred approach.

Experience in community mental health, psychosocial disability, housing and homelessness, justice, child and family services, AOD, NDIS or regional services will be highly regarded.

This role will initially be offered as a 3 or 6 month contract commencing, with the intention that, where there is a good mutual fit and ongoing program requirements, the position would generally transition into permanent full-time employment.
