Job summary

Bethanie Group is seeking a Case Manager for Home Care in Western Australia. You will coordinate services for a portfolio of clients from Claremont to Yokine, using a wellness and reablement approach to support independence and quality of life.

The role is permanent, with a hybrid schedule including on-site days at our Gwelup office and client visits, plus some work-from-home days. You will work within a collaborative team to deliver outstanding client outcomes.

Qualifications

• Experience in case management and/or care coordination in an aged care environment.
• Strong understanding of person-centred care practices.
• Experience managing client budgets and service delivery.
• Excellent communication and relationship-building skills.
• Experience leading, supporting and developing Support Workers or other frontline teams.
• Strong computer skills across Microsoft Office and client management systems.
• A current driver's licence.
• The ability to work collaboratively within a multidisciplinary team environment.

• Conduct assessments, care planning and regular reviews using a wellness and reablement approach.
• Monitor Home Care budgets and ensure monthly financial compliance and referrals.
• Coordinate with Clinical Nurses, Allied Health professionals and Support Workers for high-quality care.
• Maintain high standards of regulatory compliance across caseload.
• Lead, mentor and support a team of Support Workers to deliver exceptional care.

As a Case Manager in our Home Care team,you'll play a key role in partnering with clients to achieve theirgoals and remain living safely and confidently at home.

Working as part of a supportive multidisciplinary team, you'llcollaborate with Clinical Nurses, Allied Health professionals,Support Workers and fellow Case Managers to deliver personalisedcare solutions that promote wellbeing, choice and dignity.

About the role

As a Case Manager, you'll coordinate and manage HomeCare services for a portfolio of clients from Claremont out toOsborne Park and across to Yokine.

You will take a holistic wellness and reablement approach tosupport independence, health and quality of life.

This is a permanent position, newly created due to thecontinued growth of our Home Care services. Working Monday toFriday during standard business hours, you'll enjoy the flexibilityto balance client visits, working from home and regular attendanceat our Gwelup office, with a minimum of one day per week basedonsite.

Reporting to the Operational Support Manager and working as partof a collaborative multidisciplinary team, you'll be supported byexperienced Case Managers, Clinical Nurses, Allied Healthprofessionals and operational teams to deliver outstanding clientoutcomes.

Key responsibilities include:

• Building trusted relationships with clients and keysupports to achieve individual goals and outcomes
• Conducting assessments, care planning and regular reviewsusing a wellness and reablement approach
• Monitoring Support at Home budgets and ensuring monthlyfinancial compliance service delivery and referrals incollaboration with clients, colleagues and externalstakeholders
• Working closely with Clinical Nurses, Allied Healthprofessionals, Support Workers and broader team members tocoordinate safe, high-quality and person-centred care
• Maintaining high standards of regulatory compliance acrosscaseload
• Leading, mentoring and supporting a team of Support Workersto deliver exceptional care and services to clients

About you

You're someone who genuinely cares about helping older peoplelive independently and confidently. You combine strong casemanagement skills with empathy, organisation and a collaborativeapproach to supporting clients, colleagues and stakeholders.

You'll bring:

• Experience in case management and/or care coordination in asimilar aged care environment
• Strong understanding of person-centred care practices
• Experience managing client budgets and servicedelivery
• Excellent communication and relationship-buildingskills
• Experience leading, supporting and developing SupportWorkers or other frontline teams
• Strong computer skills across Microsoft Office and clientmanagement systems
• A current driver's licence

• You work well with others, value different professionalperspectives and contribute positively to team culture.

It would be great if you also have:

• Knowledge of Support at Home and CHSP guidelines
• Understanding of government-funded aged care services
• Experience working with culturally diverse communities
• Experience working alongside Clinical Nurses, Allied Healthprofessionals or other multidisciplinary teams
