Job summary

The Salvation Army in Brisbane is seeking a compassionate Case Manager to support individuals and families experiencing homelessness and housing instability across Brisbane North and Redcliffe.

You will conduct assessments, provide crisis intervention, and deliver trauma-informed, strengths-based casework to empower clients toward housing stability and wellbeing in a collaborative, community-focused environment.

Qualifications

• Qualifications in Community Services, Social Work or a related field.
• Experience in case management, homelessness or community services.
• Strong assessment, communication and relationship-building skills.
• Passion for supporting vulnerable people through a strengths-based approach.
• Driver’s licence and Working with Children Check and National Police Check.

Responsibilities

• Provide compassionate and professional support to individuals and families experiencing homelessness or housing instability through phone, face-to-face and walk-in services.
• Conduct comprehensive assessments, identify risks and vulnerabilities, develop safety plans, and connect clients with appropriate internal and external support services.
• Deliver person-centred, trauma-informed and strengths-based case management that empowers clients to achieve sustainable housing and improved wellbeing outcomes.
• Work collaboratively with clients to develop, implement and review tailored case plans, supporting them to achieve their goals and build long-term independence.
• Advocate on behalf of clients and coordinate access to housing, financial assistance, health, education, employment and community support services.
• Build positive relationships with stakeholders, service providers and community partners to strengthen referral pathways and improve outcomes for clients.
• Maintain accurate client records, case notes and reporting requirements while ensuring compliance with organisational standards and funding obligations.
• Participate in case reviews, supervision, team meetings and continuous improvement initiatives to support service excellence and best-practice delivery.

The Salvation Army is seeking a compassionate and resilient Case Manager to support individuals and families experiencing or at risk of homelessness.

In this rewarding role, you’ll provide assessments, crisis intervention, case management and practical support that empowers people to achieve housing stability, strengthen their wellbeing and build brighter futures.

This role requires working across Brisbane North and Redcliffe.

What You’ll Be Doing

What You’ll Bring
