Job summary

Silverchain seeks Registered Nurses or Allied Health professionals to join a care coordination team covering the Eden, Bega and Moruya NSW region. The role focuses on care navigation, health coaching and developing personalised care plans for clients with chronic conditions.

You will work in home-based settings and via telehealth, delivering high-quality, patient-centred care. Extensive orientation and ongoing training are provided, with a supportive team environment and opportunities to grow

Qualifications

• Bachelor of Nursing or Allied Health qualification (required).
• Current AHPRA registration is required.
• Experience in delivering chronic condition management, health care navigation and coaching.
• Strong communication, written and listening skills for liaising with clients and health professionals.
• Excellent time management and organisational skills to plan and prioritise work.
• Ability to work autonomously and within a team.

Responsibilities

• Coordinate care, navigate services and provide health coaching via face-to-face and telehealth to clients with chronic conditions.
• Develop individualised care plans and refer to other providers as needed.
• Support self-management principles across multidisciplinary services.

• Bachelor of Nursing or Allied Health qualification
• Current AHPRA registration.

• Demonstrated time management and organisational skills to effectively plan and prioritise work

The role

• We are seeking experienced Registered Nurses or Allied Health professional who will be responsible for care coordination, navigation, and heath coaching services via face to face andtelehealth to clients with chronic conditions.
• This role will involve developing individualised care plans, referring to other providers as needed, navigation across multi-disciplinary services and supporting self-management principals.
