Job summary

Moran Health Care Group is seeking a Care Planning Coordinator to join the Clinical Leadership team at our Moran Home in Kellyville, NSW. The role reports to the Care Manager and focuses on clinical assessment programs and tailored care plans for residents.

You will work with a dedicated team to ensure assessments inform care plans, with opportunities to contribute to clinical governance and priorities. Training and AN-ACC education will be provided.

Qualifications

• Registered Nurse with at least 2 years clinical experience in aged care.
• Knowledge of Care Planning, Assessment and Documentation in Residential Aged Care.
• Excellent verbal and written communication skills.
• Proficient with computers and health information systems.
• Current Police Check and appropriate Working Rights.
• Experience in a similar position in aged care is desirable.
• Management qualifications are highly regarded.

Responsibilities

• Establish a Care Plan review program with Care Manager to keep plans and assessments current.
• Develop training plans to ensure staff can competently complete assessments and documentation.
• Monitor use of psychotropic medications and antimicrobial stewardship, with reporting obligations.
• Plan care for new admissions and ongoing residents in consultation with the Care Team and other health professionals.
• Ensure care delivery aligns with the care plan and dignified decision-making.
• Review care incidents and modify care plans as needed with the Clinical Care Team.
• Maintain accurate client records and case notes as directed.

We are currently seeking a committed Care Planning Coordinator to join our Clinical Leadership team at our Moran Home at Kellyville.

This Monday to Friday position offers the opportunity to work closely with clinical leaders, influence resident outcomes and contribute to best-practice care in a supportive, professional environment.

Reporting to the Care Manager you will be responsible for the overall clinical assessment program, and supporting the RNs as required to ensure resident assessments are completed and inform the development of care plans tailored to resident's individual assessed care needs. As a member of the Clinical Leadership team, participates in clinical governance activities, including planning and implementing clinical priorities.

You will be supported and mentored to ensure you thrive in your role with us. You will work with a great team of dedicated people and your contribution will make a difference to our residents' enjoyment of their life every day. Key Duties:

• Working closely with the Care Manager (CM) you will establish a Care Plan review program ensuring care plans and assessments remain current and relevant, reviews are initiated when required and residents assessed care needs are well documented.
• Participating in the development of training and education plans to ensure employees are provided with the required guidance and education to enable them to competently complete assessments, care plans and documentation based on accurate and comprehensive assessment, consultations, observation, and collaboration.
• Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy and antimicrobial stewardship and reporting and consent obligations.
• Assessment and care planning for new admissions and existing residents, in consultation with internal and external stakeholders, Care Team, health professionals and Admissions Coordinator to ensure a personalised care plan is developed.
• Monitor the delivery of care is consistent with the care plan and goals including dignity of risk choices and take action as required if gaps or inconsistencies are identified.
• Review of care incidents and care change patterns for residents and collaborate with Clinical Care Team to determine if further assessment and care planning modifications are needed, initiate care plan reviews and re‑assessments.
• maintain accurate client records, case notes and other data as directed and required.

Skills/Requirements:

• Registered Nurse with at least 2 years clinical experience; aged care management experience and familiarity with the Quality Standards for Aged Care.
• A high level of knowledge and comprehension of the Care Planning, Assessment and Documentation in Residential Aged Care.
• Excellent and demonstrated verbal, written and communication skills.
• Knowledge and ability to use computers and technology efficiently.
• Current Police Check and appropriate Working Rights.
• Experience in a similar position in Aged Care - desirable.
• Management qualifications – highly regarded.
