Job summary

Moran Aged Care is seeking a Care Planning Coordinator to join the Clinical Leadership team at Moran Home in Kellyville. This Monday to Friday role works closely with clinical leaders to influence resident outcomes and support best-practice care in a supportive, professional environment.

You will coordinate the overall clinical assessment program, assist RNs to ensure timely assessments, and contribute to governance while developing personalised care plans tailored to each resident’s needs.

Qualifications

• Registered Nurse with at least 2 years clinical experience in aged care.
• Familiar with Quality Standards for Aged Care and care planning.
• Strong verbal and written communication skills; proficient with computers.
• Current Police Check and appropriate Working Rights; medical and nursing credentials.

Responsibilities

• Establish and review a Care Plan program ensuring plans and assessments are current.
• Develop training plans to enable staff to complete assessments and documentation.
• Analyze psychotropic use, antipsychotics, polypharmacy and antimicrobial stewardship.
• Coordinate care planning for new admissions and residents via collaboration.
• Monitor care delivery against the plan and address gaps or changes.
• Maintain accurate client records and documentation.

About the opportunity

We are currently seeking a committed Care Planning Coordinator to join our Clinical Leadership team at our Moran Home at Kellyville.

This Monday to Friday position offers the opportunity to work closely with clinical leaders, influence resident outcomes and contribute to best-practice care in a supportive, professional environment.

Reporting to the Care Manager you will be responsible for the overall clinical assessment program, and supporting the RNs as required to ensure resident assessments are completed and inform the development of care plans tailored to resident’s individual assessed care needs. As a member of the Clinical Leadership team, participates in clinical governance activities, including planning and implementing clinical priorities.

You will be supported and mentored to ensure you thrive in your role with us. You will work with a great team of dedicated people and your contribution will make a difference to our residents’ enjoyment of their life every day. Key Duties:

• Working closely with the Care Manager (CM) you will establish a Care Plan review program ensuring care plans and assessments remain current and relevant, reviews are initiated when required and residents assessed care needs are well documented.
• Participating in the development of training and education plans to ensure employees are provided with the required guidance and education to enable them to competently complete assessments, care plans and documentation based on accurate and comprehensive assessment, consultations, observation, and collaboration.
• Responsible for the completion of regular analysis of the use of psychotropic, antipsychotic, polypharmacy and antimicrobial stewardship and reporting and consent obligations.
• Assessment and care planning for new admissions and existing residents, in consultation with internal and external stakeholders, Care Team, health professionals and Admissions Coordinator to ensure a personalised care plan is developed.
• Monitor the delivery of care is consistent with the care plan and goals including dignity of risk choices and take action as required if gaps or inconsistencies are identified.
• Review of care incidents and care change patterns for residents and collaborate with Clinical Care Team to determine if further assessment and care planning modifications are needed, initiate care plan reviews and re-assessments.
• maintain accurate client records, case notes and other data as directed and required.

Skills/Requirements:

• Registered Nurse with at least 2 years clinical experience; aged care management experience and familiarity with the Quality Standards for Aged Care.
• A high level of knowledge and comprehension of the Care Planning, Assessment and Documentation in Residential Aged Care.
• Excellent and demonstrated verbal, written and communication skills.
• Knowledge and ability to use computers and technology efficiently.
• Current Police Check and appropriate Working Rights.
• Experience in a similar position in Aged Care - desirable.
• Management qualifications – highly regarded.
