South Eastern Community Connect in Sydney is seeking an experienced Care Partner to support aged clients in the community from Eastlakes/Mascot. The role is hybrid, full-time or contract, with sponsorship available for the right candidate.

You will work as part of a warm team, providing end-to-end case management, intake, care plan updates and budget monitoring, visiting clients at home as needed.

Formación

• Minimum of two years experience in Aged Care.
• Experience as a case manager in Aged Care is preferred.
• Own car and a valid driving license with insurance.
• Strong client-focused case management skills.
• Knowledge of Aged Care Quality Standards and regulatory requirements.
• Excellent communication and organisational abilities.

South Eastern Community Connect (SECC), a not-for-profit organisation serving the LGAs of Bayside, Cityof Sydney, Randwick, Woollahra and Waverley is looking for an experienced full-time Case Manager tosupport our aged clients in the community.

The Support at Home program provides services to aging clients to help them live independently, supported and empowered within their own home and community.

As our Care Partner, you’ll be working with a warm, dynamic team in Eastlakes/Mascot, Sydney.

About you:

• A minimum of two years experience in Aged Care
• Prior experience as a case manager, ideally within the Aged Care Sector
• Have your own car and current driving license (with at least Third-Party vehicle insurance)
• Experience in client-focused case management within your respective discipline
• Sound understanding of the aged care sector, quality standards, principles and regulatory requirements
• Strong interpersonal and communication skills
• Empathy & Compassion, the ability to connect with clients and understand their perspective
• Excellent organisational skills, attention to detail, and problem‑solving capabilities
• Accurate and timely logging of client interactions, service updates and referrals

About the role

As a Care Partner with SECC, you'll contribute to efficient and effective case management, coordinating services for clients receiving personalised support at home. You'll play a key role in . You will be involved in end to end care management, including intake, maintaining and updating care plans, monitoring participants needs, and individual budgets, and ensure services remain within allocated funding. You will be expected to visit clients at home from time to time. Our approach aligns with the Aged Care Quality Standards, ensuring best practices in every aspect of our service.
